Foreigners on spouse visas are not even allowed to get BPJS unless they are on the same KK as their wife,
In my view , BPJS officer makes a mistake by allowing foreigners in that situation (see below) . This mistake may be happen because BPJS Health has a rule that a registration for Indonesians must be done for all persons listed in a KK .

From Perpres no.59 Year 2024 ABOUT HEALTH INSURANCE
Article 1
2. Participants (of BPJS Health Program) are all people, including foreigners who have worked for at least 6 months in Indonesia, who have paid Health Insurance Contributions.
and since about 2018 it has been compulsory to have separate KKs.
Do you have the rule about it ? I did a quick search but didn't find anything about how the KK should be in a mixed marriage situation .

What I know by experience is that I got a KK in 2013 separated from my Indonesian wife's KK .

You can have separate or joint KK with spouse as a foreigner. In case that you have a joint one, spouse is the Head of the Family (Kepala Keluarga). You can get BPJS in that situation even if you are not working, as a dependant.

Well, not sure how that’s gonna work. I just looked at mine and it says I’m the head of the family 😂😂 occupation Bapak Rumah Tanggah(well it says ‘Other’ but that was what my wife told them)

You don't get a choice. The catatan sipil tells you to get (for example in S Jakarta) separate ones, and that's what you get.
 
My WNI wife had me put on the keluarga document and am now with BPJS. I am retired and do not work.
