While fish can contain salmonella and escherichia coli, it is usually deep fried in Indonesia and eliminated. Bali Belly probably comes from salmonella and e.coli from human feces (the cook did not wash his/her hands after the toilet job basically).
It is not common. But the salary is sometimes divided in low basic salary +food allowance + transport allowance, so if you are non performing employee they will put ypu to the low basic salary before being laid off.
Middle class in Indonesia starts from salary of around 2 million Rp/person by Government's definition. So all these guys working in parkings are middle class.
You can make jokes but Bali is always in top something islands in the world despite garbage and overcrowding. Competition is fierce in the F&B sector, with excelent chefs, cooks, both foreign and locals. I have not eaten burger or pizza in Bali that is bad, while I got poisoned in Koi Kemang...

Local people come in many shape and forms -poor, rich, middle classs, and in Bali they all have access to everything for everybody's budget.

Irrelevant. In 1950-60s Palangkaraya was a candidate for the national capital. Before that Yogyakarta, Bandung etc. Still, Jakarta is already 400 years+ the capital.
But when Indonesian government plays that game there are tens or hundreds of thousands of layoffs in other sectors because of irrrational economic and foreign investment policies that chase out investors, for one Apple that will employ 1.000 people.
