There is no requirement on the immigration website for a spouse kitap/kitas holder to have insurance.
The only category that mentions insurance is the retirement visa.
It's the most current on the General Immigration site. I would go with that and not question everything.Updated last?

Oh … it is just so complicated..No but citizens pick and choose whether to get vaccinated and thus bring their country down to a low risk percentage and therefore more welcome to other countries. Which is why even you as Indonesian still needs to quarantine and be fully vaccinated even before you come home. Because you live in USA. Nothing to do with your passport. You could be Nigerian living in Liechtenstein and you can still enter Indonesia now as a tourist. But you living in America cannot. Understand now!?
