I think DitJen in Jakarta is pretty backed up at the moment. Although my case is a different (kitap and not citizenship), I've been waiting 5 weeks for them to approve my kitap and was told by the KanIm counter staff that there are several people who applied before me still waiting too.
The 6.75jt is inclusive of the 2 year MERP. I didnt exactly request it, I assumed it was just part and parcel of the process. From my viewpoint, less future trips to immigration the better.
No, I just went to my local KanIm and they said I could apply over the counter. They gave me the application forms and I went back a few days later with all my documents.

Same happened to me with my watch. Went to the authorised dealer in Grand Indonesia and was told the turnaround would be 6 months just for a small repair.
