A problem with a healthcare insurance company

OK, but the top cost for BPJS Kesehatan is Rp150,000 per month and part of that is supposed to be covered by the employer. What does the remaining Rp450,000 go towards?
5% from the salary,4% paid by the employer, 1% paid by the employee. Limited to max 600.000 Rp total. Insurance is valid for employee and all his dependants(wife and kids).

Independent payment (if you pay by yourself) of BPJS is in classes, max 150.000/person.

The calculation for 'BPJS Kesehatan' premium for employed individuals is capped at IDR12m salary, meaning the maximum premium is Rp600k pm and will include their dependants. However it is important to note that some individuals, particularly younger people, may earn IDR 12m per month (pre-tax) and may not have dependants. This includes foreign workers on work permits who, if their families remain in their home countries, or already have private health insurance with better coverage will not be utilizing 'BPJS Kesehatan'.

Moreover, in cases where both partners in a marriage are employed, they each pay separately through automatic salary deductions. These groups paying Rp600k each (via personal and company contribution), while still in young ages, including foreign workers are most likely contribute partially to subsidizing the system. However, the majority of the subsidy still comes from the government, funded by taxpayers.
